64B4-4. Fee Schedule |
Effective on Tuesday, August 7, 2018
1(1) The fees for application and initial active status licensure are:
12$100 application fee and $75 initial licensure fee for a total of $175.
25(2) The initial licensure fee is refundable only if the applicant is ruled ineligible for licensure and the applicant submits a written request to the Department for a refund.
