64B6-4.009. Inactive and Delinquent Status Fees

Effective on Tuesday, February 9, 2016

1The fees for individuals holding a license pursuant to Section 11484.045, F.S., 13shall be as follows:

17(1) The fee for an inactive status license shall be $375.00.

28(2) The fee for processing a licensee’s request to change licensure status at any time other than at the beginning of a licensure cycle shall be $375.00.

55(3) The fee for delinquent status shall be $375.00.

64(4) The fee for reactivation of an inactive license shall be $150.00.

76(5) The fee for retirement status of an active or inactive license shall be $50.00.
