64B7-25.001. Examination Requirements

  • 1(1) The Department shall issue a license by examination to a person who:

    14(a) Pays to the Department the initial licensure fee set forth in subsection 2764B7-27.100, 28F.A.C.;

    29(b) Submits a completed application on form DH-MQA 1115, “Massage Therapist Licensure Application,” (Rev. 7/16).     90(c) Provides proof of graduation per Rule 9764B7-32.002, 98F.A.C., from a Board-approved massage school as defined in Section 108480.033(9). 109F.A.C.;

    110(d) Presents certification to the Board of successful completion of an approved examination for licensure; and,

    126(e) Submits to the background screening provisions as set forth in Section 138456.0135, F.S.

    140(2) Prior to the practice of colon hydrotherapy, any licensed massage therapist or massage therapy apprentice must:

    157(a) Submit a completed application entitled Application for Colon Hydrotherapy Upgrade to Massage Therapist License with Instructions, form DH-MQA 1247 (Rev. 01/13).     220(b) Provide proof of completion of a course of study in colonic irrigation at a Board approved massage school per Rule 24164B7-32.005, 242F.A.C., or a course of study equal to or more stringent than the course of study in colonic irrigation in subsection 26364B7-32.005(3), 264F.A.C., with an approved provider or school in another state, or a colonic irrigation apprenticeship per Rule 28164B7-29.007, 282F.A.C.; and,

    284(c) Present certification to the Board of successful completion of an approved examination for colonic irrigation.

    300(3) The Board approves the following examinations for licensure:

    309(a) The Massage and Bodywork Licensing Examination administered by the Federation of State Massage Therapy Boards;

    325(b) National Certification Board for Therapeutic Massage and Bodywork Examination;

    335(c) National Certification Examination for Therapeutic Massage;

    342(d) National Exam for State Licensure option administered by the National Certification Board for Therapeutic Massage and Bodywork.

    360(e) The state examination in Florida, offered prior to 1996.

    370(4) The Board approves the following examination for certification in colonic irrigation: The National Board for Colon Hydrotherapy Examination.
