64B7. Board of Massage Therapy |
64B7-28. Biennial License Renewal |
1Each massage therapist must renew their license on or before August 31 of every odd year in accordance with subsection 2164B-9.001(4), 22F.A.C.
23(1) To renew a massage therapist license, the licensee must:
33(a) Have completed the continuing education which was required during each biennial cycle as specified in Rule 5064B7-28.009, 51F.A.C., or elect inactive status at the time of renewal, or hold a license that is already in inactive status, and
72(b) Submit the appropriate form as required by subsection 8164B-9.001(1), 82F.A.C., and
84(c) Submit the renewal fee for massage therapists specified in Rule 9564B7-27.100, 96F.A.C.
97(2) A license that is not renewed in accordance with subsection 10864B-9.001(4), 109F.A.C., and these rules shall be considered delinquent as provided in Section 121456.036(5), F.S.
123(3) A license that is not renewed in accordance with subsection 13464B-9.001(4), 135F.A.C., and these rules for two consecutive biennial cycles shall be considered null and void without further action by the Board as provided in Section 160456.036(6), F.S.
162(4) A licensee whose license is in active status may choose inactive status as provided in Section 179456.036(4)(a), F.S.
181(a) To choose inactive status at the time of renewal, the licensee must submit the appropriate form as required by subsection 20264B-9.001(1), 203F.A.C., the renewal fee, and if delinquent, the delinquent license fee specified in Rule 21764B7-27.100, 218F.A.C.
219(b) To choose inactive status at any other time, the licensee must submit a written request, along with the change in status fee and if delinquent, the delinquent license fee specified in Rule 25264B7-27.100, 253F.A.C.
254(5) A licensee may choose retired status as provided in Section 265456.036(4)(b), F.S.
267(a) To choose retired status at the time of renewal, the licensee must submit the appropriate form as required by subsection 28864B-9.001(1), 289F.A.C., and the retired status fee specified in Rule 29864B7-27.100, 299F.A.C.
300(b) To choose retired status at any other time, the licensee must submit a written request, along with the retired status fee and change in the status fee specified in Rule 33164B7-27.100, 332F.A.C.
