64B8-3.008. Payment for Duplicating Licenses, Certifications, and Permits

There is no requirement of law that the board provide duplicate copies of licenses, certifications, or permits. If, however, a licensee or holder of a certificate or permit wishes to request the board to provide a duplicate license, certificate, or permit, as a courtesy and convenience to the licensee or holder of a certificate or permit, the board will grant the request if it is in writing and accompanied by a payment of $25.00.
