64B8-8.004. Permanent Revocation

Effective on Thursday, November 4, 1993

1When a license is permanently revoked, as authorized by Section 11456.07212(2)(b), F.S., the former licensee shall not be eligible for reinstatement, nor shall the former licensee be eligible to reapply for licensure in Florida.
