64B9-6.004. Retired Licensure Status

  • 1(1) A licensee wishing to change to retired licensure status during the renewal period must pay the retired license fee. If the change to retired licensure status is outside the renewal period, the change of status fee shall also be paid. 42These fees are set forth in Rule 4964B9-7.001, 50F.A.C.

    51(2) 52A retired status license may be reactivated upon accessing the licensee’s account through the on-line portal at http://www.flhealthsource.gov  or contacting the Board via MQA.NursingAppStatus@flhealth.gov, 76payment of the fees 80as set forth in Rule 8564B9-7.001, 86F.A.C., 87and compliance with the provisions of Section 94456.036(11), F.S.

    96(3) Any licensee applying for an active status license who 106does not currently hold an active license in good standing in another state, and the license has been in 125retired status for five (5) years or more, 133will be required to complete a nursing remedial course as described in Rule 14664B9-3.0025, 147F.A.C., with clinical component appropriate to the licensure level of the licensee. The remedial course must be given at a Board-approved program, and must include at least 80 hours of classroom instruction and 96 hours of clinical experience in medical/surgical nursing and any specialty area of practice of the licensee.
