64D-3.048. COVID-19 Vaccine Reporting Requirements

  • 1(1) All health care practitioners licensed 7under Chapter 458, 459 or 464, F.S., and all other enrolled COVID-19 vaccine providers, must report the following vaccination data elements in Florida SHOTS within 72 hours of administration to an individual of any dose of a COVID-19 vaccine that has been approved by or received Emergency Use Authorization from the U.S. Food and Drug Administration.

    69(b) Administered at location: type

    74(c) Administration address (including county)

    79(d) Administration date

    82(e) CVX (Product)

    85(f) Recipient race

    88(g) Recipient ethnicity

    91(h) Lot number: unit of use and/or unit of sale

    101(i) MVX (manufacturer)

    104(j) Recipient address

    107(k) Recipient date of birth

    112(l) Recipient name

    115(m) Recipient sex

    118(n) Sending organization

    121(o) Vaccine administering site (on the body)

    128(p) Vaccine expiration date

    132(q) Vaccine route of administration

    137(2) This rule will be reviewed and repealed, modified, or renewed through the rulemaking process five years from the effective date.
