1The following are prohibited:
5(1) Failure to conduct required sampling or testing, or falsification of results.
17(2) Intentionally or otherwise introducing a contaminant determined to pose a health hazard into a Limited Use, Multi-family, or Private Water System or its source.
42(3) Failure to meet schedules for compliance or corrective actions.
52(4) Failure to conduct required public notification or corrective action.
62(5) Impersonating a Department employee.
