64F-23.001. Informed Consent; Printed Materials
1(1) The Department incorporates by reference the forms titled “Fetal Development and Alternatives to Terminating a Pregnancy” to fulfill the requirements of Section 24390.0111(3), F.S. 26These forms are available in English (Form DH 150-997, effective date 10-12), Spanish (Form DH 150-998, effective date 10-12), and Haitian-Creole (Form DH 150-999, effective date 10-12).
53(2) English and Spanish versions of “Fetal Development and Alternatives to Terminating a Pregnancy” will be shipped by the Department of Health in bulk to each Florida licensed abortion clinic.
