64F-3.004. Healthy Start Care Coordination System

  • 1The Healthy Start care coordination system will assure:

    9(1) Funding is available to support the CHD screening infrastructure specified in subsections 2264F-3.002(1)-23(6), F.A.C.

    25(2) Within available funding, provision of care coordination and enhanced services as specified in Rules 4064F-3.005 41and 4264F-3.006, 43F.A.C.

    44(3) Care coordination and enhanced service providers maintain ledgers, records, and documents in accordance with generally accepted accounting procedures and practices which sufficiently and properly reflect all expenditures of funds provided through the auspices of the Coalition or the CHD.

    84(4) The provider of care coordination services will:

    92(a) Receive and review screening reports from the State Health Office and reconcile discrepancies in screening data.

    109(b) Provide ongoing quality control for data related to Healthy Start screening and care coordination as specified by the department.

    129(5) Quality improvement systems are in place to assure care coordination and enhanced services are provided according to the local service delivery plan.

    152(6) Management information system data as specified by the department is available for evaluation and accountability requirements.

    169(7) A network of providers participating in Healthy Start prenatal and infant risk screening within the community is established and maintained.

    190(8) Information and presentations related to the Healthy Start Care Coordination System are provided to the community.

    207(9) Development or review of local protocols and procedures for Healthy Start risk screening and care coordination service delivery.

    226(10) Development or review of local written procedures in collaboration with CMS Early Steps or Neonatal Intensive Care Units (NICU) staff addressing care coordination of participants admitted to CMS Early Steps or NICU.

    259(11) Care coordination and enhanced service providers will comply with client and service reporting requirements as specified by the department.
