64I-2. Volunteer Health Care Provider Program |
1(1) In order to participate in this program, a health care provider shall comply with the following:
18(a) Have a current valid Florida health professional license or authorization to practice or operate under Florida law.
36(b) Sign the Volunteer Health Care Provider Program contract, DH 1029 (12/14), with the governmental contractor. The contract is hereby incorporated by reference and available at 62https://www.flrules.org/Gateway/reference.asp?No=Ref-0546864.
65(c) Not be under obligations, probation, or restrictions with the Department of Health or any Florida licensing authority.
83(d) Submit to a credential verification process to determine acceptability of participation.
95(e) Participate in a quality assurance program as delineated by the governmental contractor.
108(f) Accept for treatment only patients that have been qualified as eligible and who have a completed referral form, which is incorporated by subsection 13264I-2.002(5), 133F.A.C.
134(2) The health care provider shall not subcontract for the provision of services under this chapter.
