  • 1(1) Licensing packets for a child-caring agency shall be uploaded into the states official system of record and include the following:

    22(a) The “License Application for Child-Caring and Child-Placing Agencies,” CF-FSP 5412, Apr 2021, 36incorporated by reference and 40available at 42http://www.flrules.org/Gateway/reference.asp?No=Ref-1306844. The application shall be signed by the licensee who is the executive director exercising authority over and has the responsibility for the operation, policies, and practices of the agency.

    74(b) Agencies providing 77care for one to six or seven to 14 children shall complete 89the “Community Residential Homes Local Ordinance Certification,” CF 981786, May 2015, incorporated by reference and available at 107http://www.flrules.org/Gateway/reference.asp?No=Ref-13065109. This application form certifies to the Department that the proposed group home meets the dispersion and notification requirements of Chapter 419, F.S.

    132(c) 133For child-caring agencies obtaining federal grants, loans, contracts, property, discounts, or other federal financial assistance, the executive director shall sign the “Civil Rights Certificate,” CF 707, Apr 2021, incorporated by reference and available at 168http://www.flrules.org/Gateway/reference.asp?No=Ref-13062170.

    171(d) 172The child-caring agency shall provide a description of the geographic area the child-caring agency serves or intends to serve, to include if serving on a county, region or statewide level, with the specific services it provides or proposes to provide. Applicants who apply for an initial license shall furnish evidence that the services will be used by referral sources or other documentation of the need for the services which shall be verified by the Department. The child-caring agency shall indicate if it plans to serve dependent and/or non-dependent populations.

    261(e) Disaster preparedness and evacuation plan. The written plan shall include the components required in subsection 65C-46.005(7), F.A.C.

    327(g) Documentation to support the safety, sanitation, and food service requirements pursuant to Rule 65C-46.005, F.A.C.:

    3431. 344Annual health and fire inspection.

    3492. Radon test results, if applicable.

    3553. Vertification of car and liability insurance.

    3624. Menus approved by the registered dietician.

    3695. Certification of registered dietician.

    374(h) 375Confirmation of local zoning notification.

    380(i) 381Verification of accreditation, as applicable.

    386(j) 387Applicant reference check from the current or previous licensing authority, if licensed in another region or state, pursuant to Rule 65C-46.011, F.A.C.

    409(2) Policies and procedures must be available for review and approved by the Department’s licensing team prior to initial licensure. Any policies or procedures that are updated during the licensing eyar must be made available for review prior to the implementaiton of proposed changes.
