69O-149.053. On-Going Review of the Self-Funded Health Benefit Plan  


Effective on Tuesday, May 18, 2004
  • 1(1) Once the Plan has been approved by the Office and has commenced operations, the local governmental unit shall submit the following, no later than 90 days after the close of the Plan’s fiscal year:

    36(a) Form OIR-B2-570, “General Information on Self-Funded Health Benefit Plans”, as adopted in Rule 5069O-149.054, 51F.A.C.;

    52(b) Form OIR-B2-572, “Annual Report of Self-Funded Health Benefit Plans”, as adopted in Rule 6669O-149.054, 67F.A.C.;

    68(c) Form OIR-B2-573, “Operating Projections for Self-Funded Health Benefit Plans”, as adopted in Rule 8269O-149.054, 83F.A.C.;

    84(d) Form OIR-B2-574, “General Information and Surplus Statement for Self-Funded Health Benefit Plans”, as adopted in Rule 10169O-149.054, 102F.A.C.; and

    104(e) A certification as to the actuarial soundness of the Plan prepared by an actuary who is a member of the Society of Actuaries or the American Academy of Actuaries accompanied by an explanation or basis of how the certification was made.

    146(2) The filing shall be submitted to the Office electronically through https://iportal.fldfs.com/.
