69B-151. Requirements For Replacement Of Life And Health Coverage |
1Where replacement is or may be involved, the agent shall:
11(1) Present to the applicant, not later than at the time of taking the application, a Form OIR-B2-312 “Notice to Applicant Regarding Replacement of Life Insurance” rev. 1-91 as described in Exhibit A adopted in Rule 4769B-151.010, 48F.A.C. The Notice must be signed by the applicant and the agent and left with the applicant.
65(2) Leave with the applicant the original or a copy of all Sales Proposals used for presentation to the applicant.
85(3) Submit to the replacing insurer with the application, a completed copy of the Form OIR-B2-312 “Notice to Applicant Regarding Replacement of Life Insurance” rev. 1-91 (Exhibit A), adopted in Rule 11669B-151.010, 117F.A.C., and a copy of all Sales Proposals used for presentation to the applicant.
