69I-22. Direct Deposit Through Electronic Funds Transfer |
1(1) “Account” means the checking or savings account identified on the Direct Deposit Authorization Form, to which the beneficiary’s payments will be transmitted. An account must be at a participating financial institution.
33(2) “Direct Deposit Authorization Form” means the appropriate Direct Deposit Authorization Form.
45(a) State employees shall use 50Form DFS-A1-26S, Employee Direct Deposit Authorization (Rev. 3/2014), 58(59http://www.flrules.org/Gateway/reference.asp?No=Ref-0490661) which is hereby incorporated by reference and is available from the Department’s website at http://www.myfloridacfo.com/Division/AA/Forms/default.htm.
77(b) Retirees shall use 81Form DFS-A1-26R, Retiree Direct Deposit Authorization (Rev. 3/2014), 89(90http://www.flrules.org/Gateway/reference.asp?No=Ref-0490992) which is hereby incorporated by reference and is available from the Florida Retirement System’s online portal at 110www.myFRS.com 111or by calling (850)907-6500 or toll free 1(844)377-1888.
119(3) “Beneficiary” means:
122(a) A person who is eligible to receive retirement benefits from the Division of Retirement of the Department of Management Services; or
144(b) A person who is eligible to receive wage payments made by the Chief Financial Officer of the State of Florida; or
166(c) A person who is eligible to receive both retirement benefits and wage payments made by the Chief Financial Officer.
186(4) “Department” means the Department of Financial Services, which pursuant to section 19820.121, F.S., 200is headed by the Chief Financial Officer.
207(5) “Participating Financial Institution” means a bank, national association, savings and loan association, mutual savings bank, or a credit union which is chartered by either any state or the federal government and authorized by the applicable federal reserve bank to receive commercial automated clearing house transactions and is thus eligible to receive State of Florida direct deposits by electronic funds transfer.
268(6) “Section” means the Direct Deposit/EFT Section, Bureau of Vendor Relations, Division of Accounting and Auditing, Department of Financial Services.
288(7) “Wages” shall have the same meaning herein as is contained in rule 30169I-31.103, 302F.A.C.
