69L-22. Reemployment Services - Workers' Compensation |
1(1) Upon approval of 5training and education by the 10Department, the injured employee and Department staff shall sign and date:
21(a) Form DFS-F3-DWC-24, 24Department and Student Agreement for Sponsorship of Training and Education, 34which is adopted in rule 3969L-22.011, 40F.A.C.; and,
42(b) An Individualized Written Reemployment Plan.
48(2) 49Upon approval of 52reemployment services, including job-seeking assistance, by the 59Department, the injured employee and Department staff must sign and date 70Form 71DFS-F3-DWC-26, 72Department and Injured Employee Agreement for the Provision of Contracted Placement Services, 84which is adopted in rule 8969L-22.011, 90F.A.C.
