69L-5.212. Contact Information Reporting

Effective on Tuesday, March 9, 2010

1Current Self-Insurers and Former Self-Insurers shall provide written notification of changes in their contact information within thirty (30) days of the effective date of the change. Notification shall be submitted as follows:
