69O-157.021. Certificate

Effective on Wednesday, May 17, 1989

1A certificate issued pursuant to a group long-term care insurance policy, which policy is delivered or issued for delivery in this state, shall include:

25(1) A description of the principal benefits and coverage provided in the policy;

38(2) A statement of the principal exclusions, reductions, and limitations contained in the policy;

52(3) A statement that the description of principal benefits is a summary of the policy and that the group master policy should be consulted to determine governing contractual provisions;

81(4) Person insured;

84(5) Person to whom benefits are payable;

91(6) Group contract number;

95(7) Certificate number;

98(8) Effective date; and

102(9) Time certificate is effective.
