  • 1Each covered employee shall receive a policy, contract, certificate, summary arrangement description, or evidence of the benefits and coverages provided. Each document shall have on the first page inside the cover of the document, or if the document does not have a cover, on the first page the statement required by Section 53624.438(2), F.S.
