  • 1(1) 2A provider or facility is accredited for the purposes of Sections 13400.235(5)(b)1. 14and 15651.105(1), F.S., 17if it is accredited without stipulations or conditions by Commission on Accreditation of Rehabilitation Facilities International (“CARF International”).

    35(2) 36The provider will file evidence that the provider or facility is accredited and in good standing each year with its annual report filing59.

    60(3) 61The provider shall furnish the Office, within five business days, a copy of any communication from CARF International concerning the loss or potential loss of accreditation of the provider or any facility owned or operated by the provider, wherever located101.
