  • 1(1) An applicant for initial registration as a pharmacy benefit manager shall submit the following:

    16(a) A nonrefundable fee of $5.00;

    22(b) A copy of the applicant’s corporate charter, articles of incorporation, or other charter document submitted electronically via the Office’s iApply system at https://www.floir.com/iportal; and

    47(c) A completed Form OIR-C1-2209, Registration Form for Pharmacy Benefit Managers, effective 01/19, hereby incorporated by reference and available at 67http://www.flrules.org/Gateway/reference.asp?No=Ref-10321, 69submitted electronically via the Office’s iApply system at https://www.floir.com/iportal.

    78(2) Each registrant renewing its registration shall submit a nonrefundable fee of $5.00 and a completed Form OIR-C1-2209 submitted electronically via the Office’s iApply system at https://www.floir.com/iportal.
