    64B-9.005Active Duty Military or Veterans Licensure.

    64B-9.007Temporary Certificate for Active Duty Military Health Care Practitioners Application.

    PURPOSE AND EFFECT:  To amend the application for active duty military and veteran licensure and provide an application form for spouses of active duty military seeking licensure in this state, and to provide for a 6-month temporary certificate for active duty military health care practitioners. 

    SUMMARY: This rulemaking amends the application form for active duty military and veteran licensure, provides an application form for spouses of active duty military seeking licensure in this state, and provides for a 6-month temporary certificate for active duty military health care practitioners, as required by recently enacted legislation. 

    SUMMARY OF STATEMENT OF ESTIMATED REGULATORY COSTS AND LEGISLATIVE RATIFICATION: The agency has determined this amendment will not have an adverse impact on small business or likely increase directly or indirectly regulatory costs in excess of $200,000 in the aggregate within one year after the implementation of the rule. A SERC has not been prepared by the agency.

    The Agency has determined that the proposed rule is not expected to require legislative ratification based on the statement of estimated regulatory costs or if no SERC is required, the information expressly relied upon and described herein: Based on the SERC checklist, this rulemaking will not have an adverse impact or regulatory costs in excess of $1 million within five years as established in s.120.541(2)(a), F.S.

    Any person who wishes to provide information regarding the statement of estimated regulatory costs, or to provide a proposal for a lower cost regulatory alternative must do so in writing within 21 days of this notice.

    Active duty military and veterans All persons seeking licensure pursuant to Section 456.024(3), F.S., shall submit to the Department a completed Florida Department of Health Active Military or Veterans Licensure Application, Form DH5000-MQA (06/2016) (5/14), which is incorporated by reference and available at https://www.flrules.org/Gateway/reference.asp?No=Ref-#####Spouses of active duty military seeking licensure pursuant to Section 456.024(3), F.S., shall submit to the Department a completed Florida Department of Health Active Military Spouse Licensure Application, Form DH5015-MQA (07/2016), incorporated by reference and available at https://www.flrules.org/Gateway/reference.asp?No=Ref-#####.

    64B-9.007 Temporary Certificate for Active Duty Military Health Care Practitioners Application.

    All persons seeking licensure pursuant to Section 456.0241, F.S., shall submit to the Department a non-refundable application fee of $50 and a completed Florida Department of Health Temporary Certificate for Active Duty Military Health Care Practitioners Application, Form DH5016-MQA (07/2016), incorporated by reference and available at https://www.flrules.org/Gateway/reference.asp?No=Ref-#####.  Temporary certificates expire after 6 months and may be renewed using Temporary Certificate for Active Duty Military Health Care Practitioners Renewal Application, Form DH5017-MQA (07/2016), incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-####.  Renewal applications must be accompanied by a renewal fee of $50.00 and evidence that the military health care practitioner continues to be a military platform participant.
