Proposals are requested from QUALIFIED REGISTERED ROOFING CONTRACTORS by the State of Florida, Department of Children and Families, hereinafter referred to as OWNER, for the construction of:

SITE: Florida State Hospital, Chattahoochee, Florida
PREQUALIFICATION: Each prime Bidder shall be state-certified in accordance with Chapter 489, Florida Statutes, as a Roofing Contractor. Bids from firms not able to furnish proof of the required certification are subject to disqualification.
PROPOSAL: Bids must be submitted in full in accordance with the requirements of the Drawings and the General and Technical Specifications, which may be examined and obtained from the ARCHITECT/ENGINEER:

MANDATORY PRE-BID MEETING: A mandatory pre-bid meeting will be held on Thursday, January 21, 2016 at 10:00 a.m. local time. The meeting will be held at Maintenance & Facilities Conference Room, Building #1123, Lonesome Pine Road on Florida State Hospital Campus, Chattahoochee, Florida.
BID OPENING: Sealed bids will be received, publicly opened and read aloud:

LOCATION: 1317 Winewood Blvd., Building #3, Room 205G, Tallahassee, Florida. Opening and reading bids aloud will occur in Building #3, Room 202, Conference Room
SUMMARY OF WORK:
All work to be performed shall be located on Unit 15/Laboratory, Building No. 1238, Florida State Hospital, Chattahoochee, Florida. The existing building (110 squares plus or minus) roof system consist of concrete structural deck with lightweight insulating concrete, 3 ply membrane system and ballast with an internal roof drainage system. The Work shall consist of removing ballast/gravel and membrane roofing system. All existing roof material to be removed shall be placed in trash chute. Install adhere attached new fleece backed fabric-reinforced 60 mill thermoplastic PVC sheet membrane (110 squares plus or minus) and install new retrofit roof drains. The manufacture that will be issuing the warranty will perform an Infrared moisture survey within 5 days of notice to proceed. Infrared will be done at dusk, no less than 48 hours after rainfall and will be done with a representative of the contractor, Architect, and building owner on site. Contractor is required to list a line item cost per board foot to remove wet lightweight concrete. Fill void with ISO board to bring to grade and then torch down new Mod Bit over repair area. Wet areas will be determined by the Infrared survey and wet areas marked on roof at time of survey.
