NOTICE IS HEREBY GIVEN that the Board of Physical Therapy Practice has received the petition for declaratory statement from Jessica D. Smith, PTA., filed on December 20, 2012.The petition seeks the agency's opinion as to the applicability of Chapter 486.021, Florida Statutes as it applies to the petitioner.
