NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 46 No. 210, October 27, 2020 issue of the Florida Administrative Register.
The changes are based upon written comments received by the staff of the Joint Administrative Procedures Committee. This notice of change supersedes the Notice of Change published on 12/29/20 in the F.A.R.
The rule now reads as follows:
64B17-7.0027 Procedure for Compliance with Board Ordered Laws and Rules Exam.
Licensees ordered to take and pass the Florida Laws and Rules Examination as a result of a disciplinary proceeding or reinstatement, must file DOH Form #DH-MQA 1144, Final Order / Continuing Education Credit Florida Laws and Rules Application, revised 12/2020, which is incorporated by reference and available through http://www.flrules.org/Gateway/reference.asp?No=Ref-_______ or www.floridasphysicaltherapy.gov/resources.

The following changes have been made to incorporated form DH-MQA 1144.
Page 4, Number 3: The hyperlink has been removed.
Page 6, Number 6: A reference to section 486.081 and 486.107 has been included in the fourth paragraph.
The revision date to the form has been changed to 12/2020.
