The Board of Dentistry hereby gives notice:

Of the issuance of an Order regarding the Petition for Waiver or Variance, filed on September 23, 2013, by Barbara Mazurkiewicz. The Notice of Petition for Waiver or Variance was published in Vol. 39, No. 210, of the October 28, 2013, Florida Administrative Register. Petitioner sought a waiver or variance of paragraph 64B5-2.0135(3)(d), F.A.C. The Board considered the instant Petition at a duly-noticed public meeting, held November 15, 2013, in Gainesville, Florida.

The Board’s Order, filed November 26, 2013, denied the petition finding that Petitioner has not met the statutory requirement of Chapter 466.007, F.S., which requires the petitioner to take and pass the ADEX examination.
