AMENDED REQUEST FOR PROPOSAL

South Florida Community Care Network, LLC d/b/a Community Care Plan (CCP) is issuing an amendment to the Request for Proposal (RFP) for Transportation Services for Medicaid Members published on September 7, 2018 in Vol. 44/175. The Receipt of Vendor Acknowledgment Form must be received by CCP no later than 5:00 p.m. (ET) on October 2, 2018.

Written follow-up questions must be received by CCP no later than 1:00 p.m. (ET) on October 2, 2018.

Responses to the RFP must be received by CCP no later than 1:00 p.m. (ET) on October 5, 2018, by sending proposals to the following: Community Care Plan, 1643 N. Harrison Parkway, Bldg. H, Suite 200, Sunrise, FL 33323

All responses will be analyzed on the basis of the criteria set forth in the Amended RFP.

The evaluation of the proposals received will be conducted from October 12, 2018 to October 19, 2018. Selected vendors will be required to present an oral presentation on October 15, 2018 at CCP. CCP requests that respondents hold this date for an on-site presentation.

The expected award date of the RFP is November 1, 2018.  The Contract for transportation services to CCP Medicaid members will begin on December 1, 2018.

CCP reserves the right to accept or reject, in whole or in part and for any reason whatsoever, any and all responses to the Request for Proposal and to waive any formalities in the response process.
