REQUEST FOR STATEMENTS OF QUALIFICATIONS
for ARCHITECTURAL AND ENGINEERING SERVICES
The Sarasota County Public Hospital Board of Sarasota, Sarasota Memorial Hospital, Sarasota County, Florida (“the Hospital”) is accepting statements of qualifications from Architectural/Engineering Consulting Firms under the provisions of the Consultants’ Competitive Negotiation Act, Section 287.055; Florida Statutes. The scope of work may include programming, schematic level design, for all architectural, civil, site work, mechanical, electrical, plumbing, fire protection and structural design work documents for a new, nine-story, approximately 175,000 square foot, comprehensive, full service Oncology Center, located at 1700 S. Tamiami Trail, Sarasota, FL 34239. The oncology specific services will include, but are not limited to, in-patient and out-patient units of care, as well as related imaging and surgical services. There will be significant connectivity, and access to the existing facility will need to remain active during all phases of the construction with a multi-phase approach with restricted contractor parking. Services required may include architectural design, civil, mechanical, electrical, plumbing, fire protection, structural design and engineering including working in tandem with a consulting construction manager. This scope of work is for schematic level design only.
Firms interested in being considered as candidates are required to submit five bound statements of qualifications that include at least the following data, to be organized in the following order:
1.A copy of Florida Architectural Licensure and corporate registration certificates.
2.Proof of Professional Liability Insurability, Errors and Omissions.
3.A separate statement as to whether the firm is a certified Minority Business Enterprise. If your firm claims MBE, WMBE status, a copy of your firm’s current, valid MBE, WMBE certificate is required as part of the submission package.
4.Proposed design team with resumes, and their AHCA experience.
5.A list of at least five client references consisting of name, title, address, telephone number and project name(s) for each project specified. These references are contacted. A Non-Answer response negates the reference.
6.Past design experience with new construction and renovations as it relates to a combination of large hospital, enabling, and tertiary projects, including roadway access. Include projects requiring a multi-phased approach.
7.Design and permitting experience within local agencies in Florida and all other applicable permitting agencies.
8.Location of the design firm’s main office.
All interested firms are further informed as follows:
1.The Hospital reserves the right to reject any or all submittals at any time during this process.
2.The basis for selecting candidates includes, but is not limited to, the firm’s experience with local regulatory agency having jurisdiction, AHCA Florida experience, consideration of related project experience, qualifications of proposed team, design criteria experience as stated above, ability to respond, and project approach.
3.The Hospital reserves the right to request additional information beyond the data set forth above.

Submittals received after this deadline will remain unopened and available for pick up.
6.Only Thomas Perigo shall be contacted with regard to this Request. No other SMH staff, administrators, or board members shall be contacted. Failure to comply could result in immediate disqualification at the discretion of the Director of Architecture and Construction.

8.The selection committee will meet in a public meeting on Thursday, October 27, 2016, in the Jacaranda Room, Waldemere Medical Plaza, 1921 Waldemere St., Sarasota, FL, 34239, from 1:00 p.m. to 3:30 p.m. to discuss and announce the top three short list firms with whom the Hospital will subsequently invite to a second public meeting on Tuesday, November 15, 2016, in Sarasota Memorial Hospital’s first floor Auditorium, 1700 S. Tamiami Trail, Sarasota, FL 34239, where all three firms will present project specific qualifications. Details of the second public meeting will be discussed with the three shortlisted firms immediately after the first public meeting.
9. Immediately after the second public meeting, the Hospital will rank and announce the ranking order. The Hospital will subsequently engage in contract negotiations. All interested parties are invited to attend.
