64ER17-5: Waiver of DH Form 680 and SHOT System for School Immunization Record – Hurricane Maria

SPECIFIC REASONS FOR FINDING AN IMMEDIATE DANGER TO THE PUBLIC HEALTH, SAFETY OR WELFARE: In response to Hurricane Maria, Governor Rick Scott issued Executive Order Number 17-259 to effect disaster response for the people of Puerto Rico who evacuate to Florida.

REASON FOR CONCLUDING THAT THE PROCEDURE IS FAIR UNDER THE CIRCUMSTANCES: For those who evacuate from Puerto Rico to Florida due to Hurricane Maria, this emergency rule provides for the waiver of DH Form 680 and SHOT System for School Immunization Record.

64ER17-5 Waiver of DH Form 680 and SHOT System for School Immunization Record - Hurricane Maria

For persons who evacuate from Puerto Rico to Florida as a result of Hurricane Maria, the Department of Health waives the requirement to use DH Form 680 and SHOT System for the school immunization record.  The Department through the Bureau of Epidemiology or applicable County Health Department may use the equivalent record provided by the United States Territory of Puerto Rico to meet the requirement(s) of 64D-3.046.
