64B8-4.009 Applications.
(1) All persons applying for licensure shall submit an application to the Executive Director on forms approved by the Board and provided by the Department. The application shall be made on Form DH-MQA 1000 (revised 08/08), hereby adopted and incorporated by reference, and can be obtained from the Board of Medicines website at http://www.doh.state.fl.us/mqa/medical/me_applicant.html. The application must be accompanied by the application fee.
