    PURPOSE AND EFFECT: The proposed amendment to Rule 59G-4.340, F.A.C., incorporates by reference in rule the revised Florida Medicaid Visual Services Coverage and Limitations Handbook, January 2010. The effect of the handbook update includes a reduction in Medicaid coverage for eyeglass frames and lenses for recipients age 21 and older and clarification of Visual Services policy.

    (2) All visual services practitioners enrolled in the Medicaid program must be in compliance with the provisions of the Florida Medicaid Visual Services Coverage and Limitations Handbook, updated January 2010 January 2006, updated January 2007, which is incorporated by reference, and the Florida Medicaid Provider Reimbursement Handbook, CMS-1500, which is incorporated by reference in Rule 59G-4.001, F.A.C. Both handbooks are available from the Medicaid fiscal agent’s Web Portal website at http://mymedicaid-florida.com http://floridamedicaid.acs-inc.com. Click on Public Information for Providers, then on Provider Support, and then on Provider Handbooks.
