NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 46 No. 138, July 16, 2020 issue of the Florida Administrative Register.
The Board made changes to the proposed form set forth in the above rule. In Rule 64B20-4.001, form DH-SPA-3, changes have been made to the form pertaining to the health history questions. The changes are only being made to the proposed forms.
