NOTICE IS HEREBY GIVEN that on October 17, 2012, the Board of Dentistry, received a petition for Miriam Halpern, seeking a variance or waiver of Rule 64B5-2.0135, Florida Administrative Code, asking the board to waive the ADEX Dental Hygiene Examination requirement, as she is currently licensed in three different states and passed the North East Regional Board exam.
