NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 46 No. 91, May 8, 2020 issue of the Florida Administrative Register in response to written comments from the Joint Administrative Procedures Committee on Rule 64B6-8.002.
The changes to the form’s revision dates are as follows:
64B6-8.002 Qualifications for Trainees, Sponsors and Designated Hearing Aid Specialists.
(1) The Department shall temporarily certify, until the next Board meeting, only those applicants who meet the following requirements, in addition to those imposed by law:
(a) The applicant shall have secured the sponsorship of an eligible sponsor;
(b) The applicant must complete the Hearing Aid Specialist Application for Training Program Registration, (Form DH-MQA 1158 (revised 07/2010/19), hereby adopted and incorporated by reference, which can be obtained from the Board of Hearing Aid Specialists’ website at http://floridahearingaidspecialists.gov/licensing/), and at http://www.flrules.org/Gateway/reference.asp?No=Ref- , and pay the appropriate fee; and,
(c) No change.
(2) through (3) No change.

Additionally, the Board made changes to the proposed form set forth in the proposed rule.
64B6-8.002(1)(b): Form DH-MQA 1158 cites chapter 64B6-8 rather than “Rule 64B6-8.” Form DH-MQA 1158 has removed requirements of affirmations by the Applicant.
