NOTICE IS HEREBY GIVEN that on October 7, 2013, the Board of Dentistry received a petition for Nancy Montgomery, DMD, seeking a variance or waiver of Rule 64B5-14.005, Florida Administrative Code, asking the board to waive the requirement for a conscious sedation permit. Petitioner is seeking a permanent waiver based on training and experience, and the Petitioner previously held a conscious sedation permit. The Board will address the Petition at its next scheduled meeting.
