- NOTICE OF CHANGENotice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 38 No. 5, February 3, 2012 issue of the Florida Administrative Register.
The change is in response to written comments submitted by the staff of the Joint Administrative Procedures Committee. The changes are as follows:
1. The first sentence of the rule shall now read as follows: An applicant, filing DOH Form #DH-MQA 1142 Application for Licensure, Revised 08/12, which is hereby incorporated by reference, which is available through www.doh.state.fl.us/mqa, and demonstrating that he or she is licensed in another state may be licensed to practice as a physical therapist assistant by endorsement by presenting evidence of active licensure in another jurisdiction, under oath, and evidence satisfactory to the Board that the applicant from such other jurisdiction has been licensed under standards for licensure as high as those maintained in Florida.
2. 486.106, 486.107, F.S. will be added to the Law Implemented.
3. In regards to the incorporated form, the $25 law exam processing fee has been removed from the application.
4. Typographical errors have been corrected in the application.
