NOTICE IS HEREBY GIVEN that on September 23, 2013, the Board of Dentistry received a petition for Barbara Mazurkiewicz, seeking a variance or waiver of paragraph 64B5-2.013(3)(d), Florida Administrative Code, asking the board to waive the requirement that the written examination (laws and rules) be completed within eighteen (18) months of completing the practical or clinical examination. The petitioner is currently licensed in New York and has passed the NERB-ADEX. The Board will address the Petition at its next scheduled meeting.
