SECOND AMENDED REQUEST FOR PROPOSAL
South Florida Community Care Network, LLC d/b/a Community Care Plan (CCP) is issuing an amendment to the Request for Proposal (RFP) for Transportation Services for Medicaid Members published on September 7, 2018 in Vol. 44/175 and October 1, 2018 in Vol. 44/191. The mandatory Vendor Acknowledgment Form must be received by CCP no later than 9:00 a.m. (ET) on October 5, 2018.
Interested vendors may participate in a Question & Answer conference call (Q&A Session) on October 9, 2018 at 3:00 p.m. (ET) by dialing 1(240)454-0879 USA Toll, ID/Access Code: 738-767-676. Interested vendors should email jmcdaniel-chinn@ccpcares.org to confirm attendance of the Q&A Session. In order for CCP to respond appropriately during the Q&A Session, interested vendors should make every effort to submit their questions in advance by email to jmcdaniel-chinn@ccpcares.org no later than 5:00 p.m. (ET) on October 5, 2018.
Written follow-up questions to the Q&A Session must be received by CCP no later than 1:00 p.m. (ET) on October 12, 2018.
Responses to the RFP must be received by CCP no later than 2:00 p.m. (ET) on October 29, 2018, by sending proposals to: Community Care Plan, 1643 N. Harrison Parkway, Bldg. All responses will be analyzed on the basis of the criteria set forth in the Second Amended RFP.
The evaluation of the proposals received by CCP will be conducted on November 1 through 6, 2018. Selected vendors will be required to present an oral presentation at 10:00 a.m. on November 9, 2018 at CCP. Respondents should hold this date for an on-site presentation.
The expected award date of the RFP is November 20, 2018. The contract for transportation services to CCP Medicaid members will begin on December 1, 2018.
CCP reserves the right to accept or reject, in whole or in part and for any reason whatsoever, any and all responses to the Request for Proposal and to waive any formalities in the response process.
