Lake Wells Medical Center emergency service exemption

The Agency for Health Care Administration has received applications for emergency service exemptions from Lake Wales Medical Center located at 410 S 11th St, Lake Wales, FL 33853 pursuant to Section 395.1041 (3), Florida Statutes and 59A-3.255, Florida Administrative Code. The hospital is requesting emergency service exemptions for Neurology, Urology, Otolaryngology, Gynecology, Ophthalmology, and Gastroenterology. Comments received within 15 days of publication will be considered by the Agency prior to making a determination of exemption status.
