(1) A claim against the Recovery Fund shall be made on Form DBPR/CILB/022, (Rev. October 2008) “Florida Homeowners’ Construction Recovery Fund Claim Form,” hereby adopted and incorporated by reference as provided in Rule 61G4-12.006, F.A.C.
