NOTICE IS HEREBY GIVEN that on November 7, 2014, the Board of Acupuncture received a petition for variance or waiver, filed by Shirlee Fazzino. Petitioner is seeking a waiver or variance of Rule 64B1-3.004, Florida Administrative Code, in regards to the requirement that an applicant must complete and pass the NCCAOM examinations, consisting of the Foundations of Oriental Medicine Module, Acupuncture with Point Location Module, Biomedicine Module and the Chinese Herbology Module.
