NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 38, No. 34, August 24, 2012 issue of the Florida Administrative Register.

(1) The Department incorporates by reference the forms titled “Fetal Development and Alternatives to Terminating a Pregnancy” to fulfill the requirements of Section 390.0111(3) F.S.  These forms are available in English (Form DH 150-997, effective date 10/12 3/2012), Spanish (Form DH 150-998, effective date 10/12 3/2012), and Haitian-Creole (Form DH 150-999, effective date 10/12 3/2012).
