    NOTICE IS HEREBY GIVEN that on October 23, 2017, the Board of Optometry, received a petition for variance or waiver filed by Ami Demebie Dechavez Casis from subsection 64B13-4.001(2), F.A.C., which states that an applicant for licensure must achieve a passing score on all four parts of the licensure examination including an overall passing score on the CSE, as well as the required score of 75 percent (75%) or better on each of the three (3) individually identified skills, on the same test attempt.
