Notice is hereby given that the following additional changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Volume 38, No. 1, of the January 6, 2012, issue of the Florida Administrative Weekly. The additional changes are being made to the application form to address several questions relating to subsection 456.0635(2), Florida Statutes.
