    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 47 No. 192, October 4, 2021 issue of the Florida Administrative Register.

    The change is in response to written comments submitted by the staff of the Joint Administrative Procedures Committee. The Board voted at the duly-noticed public board meeting held August 25, 2021, in Tampa, Florida, to amend the rule. The rule shall now read as follows:

    64B16-26.1032 Immunization Administration Certification Application for Pharmacists & Pharmacy Interns.

    All applications for immunization certification shall be made on board approved form DH-MQA 1125, “Immunization Administration Certification Application for Pharmacists & Pharmacy Interns,” 08/2021 06/2021, which is hereby incorporated by reference. To obtain an application, contact the Board of Pharmacy at 4052 Bald Cypress Way, Bin #C04, Tallahassee, FL 32399-3254, or (850) 245-4474, or download the application from the Department of Health’s website at http://floridaspharmacy.gov, or at http://www.flrules.org/Gateway/reference.asp?No=Ref-       08566. The application must be accompanied with a non-refundable application fee, if applicable, as set forth in Rule 64B16-26.1001, F.A.C.

    The following changes have been made to incorporated form DH-MQA 1125:

    •                               On page 4 of 4, #7 (third paragraph) has been amended to read, “I am aware that my immunization administration certification may be suspended or revoked if I violate any provision of Chapter 456, Chapter 465, and/or any laws or rules adopted pursuant thereto.”

     

    64B16-26.203 Pharmacist Licensure by Examination (U.S. Graduates); Application.

    Applicants who are at least eighteen (18) years of age and a recipient of a degree from a school or college of pharmacy accredited by an accrediting agency recognized and approved by the United States Department of Education may apply to take the licensure examination.

    (1) All applications for licensure by examination must be made on board approved form DHMQA 101, Pharmacist Examination Application for U.S. Graduates, 08/2021 06/2021, which is hereby incorporated by reference, and which can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-          07403, the Board of Pharmacy, 4052 Bald Cypress Way, Bin #C04, Tallahassee, Florida 32399-3254, or the Board’s website at http://floridaspharmacy.gov. The application must be accompanied with an examination fee and an initial license fee as set forth in Rules 64B16-26.1001 and 64B16-26.1002, F.A.C.

    (2) through (4) No change.

    64B16-26.2031 Licensure by Examination (Non-U.S. Graduates); Application.

    In order for a foreign pharmacy graduate to be admitted to the professional licensure examination, the applicant must be a graduate of a four year undergraduate pharmacy program at a school or college outside the United States and have completed an internship program approved by the Board.

    (1) All applications for licensure by examination must be made on form DH-MQA 103 (Rev. 08/2021 06/2021), Pharmacist Examination Application For Non-U.S. Graduates, which is hereby incorporated by reference, and which can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-         07404, the Board of Pharmacy, 4052 Bald Cypress Way, Bin #C04, Tallahassee, Florida 32399-3254, or the Board’s website at http://floridaspharmacy.gov. The application must be accompanied with an examination fee and an initial license fee as set forth in Rules 64B16-26.1001 and 64B16-26.1002, F.A.C.

    (2) No change.

    64B16-26.204 Pharmacist Licensure by Endorsement; Application.

    (1)(a) U.S. Graduates. All applications for licensure by endorsement for pharmacists who received a degree from a school or college of pharmacy accredited by an accrediting agency recognized and approved by the United States Department of Education shall be made on board approved form DOH-MQA100, 08/2021 06/2021. Pharmacist Licensure by Endorsement Application for U.S. Graduates, which is hereby incorporated by reference, and which can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-       07752, the Board of Pharmacy, 4052 Bald Cypress Way, Bin #C04, Tallahassee, Florida 32399-3254, or the Board’s website at http://floridaspharmacy.gov.

    (b) Foreign Graduates. All applications for licensure by endorsement for pharmacists graduated from a pharmacy program of a school or college of pharmacy located outside of the United States shall be made on board approved form DOH-MQA 1196, 08/2021 06/2021, Pharmacist Licensure by Endorsement Application for Non-U.S. Graduates, which is hereby incorporated by reference, and which can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-            07753, the Board of Pharmacy, 4052 Bald Cypress Way, Bin #C04, Tallahassee, Florida 32399-3254, or the Board’s website at http://floridaspharmacy.gov.

    (2) through (5). No Change.
