NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 42 No. 180, September 15, 2016 issue of the Florida Administrative Register.

Form DH5000-MQA was changed to correct coding; amend the effective date of the form to July 2016; add a reference to chapter 459 to listings for physician assistants and anesthesiologist assistants; include Certified Nursing Assistants on the list of professions requiring background screenings; and, amend the financial responsibility form for dentists, midwives, and advanced registered nurse practitioners to reflect the financial responsibility forms adopted by the licensing boards.  Form DH5015-MQA was changed to correct coding; add a reference to chapter 459 to listings of physician assistants and anesthesiologist assistants; include Certified Nursing Assistants on the list of professions requiring background screenings; and, amend the financial responsibility forms for midwives, advanced registered nurse practitioners to reflect the financial responsibility forms adopted by the licensing boards.  Form DH5016-MQA was changed to add a reference to chapter 459 to listings for physician assistants and anesthesiologist assistants; and, to include Certified Nursing assistants in the list of professions requiring background screenings.
