Any person who wishes to provide information regarding a statement of estimated regulatory costs, or provide a proposal for a lower cost regulatory alternative must do so in writing within 21 days of this notice.

64B16-29.002 General Requirements.
(1) Application for an Animal Control Shelter Pharmacy permit shall be made on Board of Pharmacy approved form DOH-MQA/PH/107 Animal Control Pharmacy Permit Application and Information, effective October 2009, which is incorporated by reference. To obtain an application, contact the Board of Pharmacy at 4052 Bald Cypress Way, Bin #C04, Tallahassee, FL 32399-3254, or (850)488-0595, or download the application from the boards website at http//www.doh.state.fl.us/mqa/pharmacy. The applicant shall apply to the Department of Health for Modified Class II Institutional Pharmacy Permit.
