PURPOSE AND EFFECT: To update, reorganize, and add questions to the licensure application in accordance with legislation passed during the 2009 Session.
SUMMARY: This rule incorporates the revised application form including questions required by Section 456.0635(2), Florida Statutes.
SUMMARY OF STATEMENT OF ESTIMATED REGULATORY COSTS: No Statement of Estimated Regulatory Cost was prepared.
Any person who wishes to provide information regarding a statement of estimated regulatory costs, or provide a proposal for a lower cost regulatory alternative must do so in writing within 21 days of this notice.

Any person desiring a license to practice school psychology either through endorsement or by examination shall apply to the Department of Health. The application shall be made on incorporated by reference form DH-MQA 1067, (11/09) (2/09) Application for School Psychology Licensure, which can be obtained from the Department of Health, 4052 Bald Cypress Way, Bin C05, Tallahassee, Florida 32399-3255 or at http://www.doh.state.fl.us/mqa/schoolpsych.
