    PURPOSE AND EFFECT: The purpose of the proposed amendments to Rule 12D-16.002 (Index to Forms), F.A.C., is to adopt new and amended forms to support the creation of new Rule Chapter 12D-19 (Florida Professional Certification Program), F.A.C. These forms include: amendments to existing Form DR-410 (Application for Florida Professional Certification; Property Tax Oversight (PTO) Certification and Training), new Form DR-410CE (Application for Approval of Continuing Education Credit Hours; Property Tax Oversight (PTO) Certification and Training), and new Form DR-410E (Course Enrollment Form; Property Tax Oversight (PTO) Certification and Training). Repeal obsolete forms (DR-516, DR-516E, and DR-591) to combine provisions into Form DR-410. The effect of the proposed changes to this rule is that property appraisers, tax collectors, employees of appraisers and collectors, and employees of the Department have forms they must use to participate in the Department’s professional certification program.
